Job Description
We are looking for experienced Home Care LPNs to join our team at ChristianaCare. As a Home Care LPN, you will have the opportunity to provide skilled nursing care and treatment to geriatric patients in the community. This role involves implementing care plans, participating in discharge planning, and ensuring the highest level of patient-centered care.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
