Job Description
We are looking for a dedicated and compassionate Registered Nurse (RN) to join our team at Atrium Health. As a Home Health Nurse, you will be responsible for providing excellent care to adult and geriatric patients in the comfort of their own homes. Responsibilities
- Manage an assigned group of patients, overseeing post-acute services in the home setting.
- Provide direct patient care, supervise LPNs and HHAs, and coordinate care with a multi-disciplinary team.
- Ensure timely care, documentation, and high-quality outcomes based on established plans of care.
Requirements
- Graduate from an accredited School of Nursing, BSN preferred.
- Current RN license with BLS certification.
- Experience in home health care is a plus.
