Registered Nurse (RN) - Home Health - $32-58 per hour

Job Description

We are looking for a dedicated Registered Nurse (RN) - Home Health to join our team at Atrium Health in Charlotte, North Carolina. This role involves providing compassionate care to adult and geriatric patients in the comfort of their own homes.

Responsibilities

• Manage an assigned group of patients, overseeing and managing post-acute services in the home setting.
• Provide direct patient care and supervise LPNs and HHAs following an established plan of care.
• Coordinate care with the multidisciplinary team and act as the patient's liaison with other healthcare providers.
• Ensure timely care, documentation, and high-quality patient outcomes.

Requirements

• Graduate from an accredited School of Nursing, BSN preferred.
• Current RN license or temporary license as a Registered Nurse Petitioner.
• BLS required per policy guidelines.
• Valid driver's license.
