Job Description
We are looking for experienced HomeHealth Licensed Practical Nurses (LPNs) to join ChristianaCare HomeHealth for full-time, part-time, and per diem positions. Our Electronic Health Record system and comprehensive OASIS-E training will support you in excelling in your HomeHealth career. You will provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in patient discharge planning.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.
