Responsibilities

• Manage a diverse patient panel inclusive of acute, chronic, and preventive care needs.
• Perform in-office and telehealth visits, as well as collaborate with virtual clinical teams.

• Demonstrate proficiency in evidence-based primary care and use technology effectively to enhance patient care.

Requirements

• Completion of FNP or PA program with national certification.
• Minimum of 2 years of primary care experience in outpatient settings.
• State licensure in New York prior to the start date.
• Passion for human-centered primary care and the ability to communicate effectively with diverse populations.
