Job Description
We are looking for an experienced Home Care LPN to join ChristianaCare's HomeHealth team. As an LPN, you will provide skilled nursing care and treatment to geriatric patients in the community, working collaboratively with the healthcare team to ensure the best possible outcomes for our patients.
Responsibilities
- Deliver skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans that align with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Must hold a current DE LPN license or compact state license.
