Job Description
We are looking for a dedicated and experienced Home Care LPN to join our team at ChristianaCare. As a Full Time, Part Time, or Per Diem LPN, you will provide skilled nursing care to geriatric patients in the community, implementing care plans and supporting patient discharge plans. Our Electronic Health Record system and comprehensive training will help you excel in your Home Health career.
Duties and Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement patient-centered care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- HSD or GED required.
- Must possess a current DE LPN license or compact state license.
