Job Description

We are looking for dedicated and experienced Licensed Practical Nurses (LPNs) to join our ChristianaCare HomeHealth team. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implementing patient-centered care plans with the support of our advanced Electronic Health Record system and comprehensive OASIS-E training.

Responsibilities

Provide skilled nursing care and treatments to geriatric patients.
Develop and implement patient-centered care plans.
Participate in discharge planning.

Requirements

High School Diploma (HSD) or GED.
Current DE LPN license or compact state license.
