Job Description
We are looking for a dedicated Licensed Practical Nurse (LPN) to join our ChristianaCare HomeHealth team. As an LPN, you will provide skilled nursing care and treatment to geriatric patients in the community. You will play a crucial role in implementing patient-centered care plans and assisting in discharge planning.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement patient-centered care plans in line with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license, or compact state license.
