Job Description
ChristianaCare HomeHealth is looking for qualified LPNs to join our dynamic team. As an LPN in our Home Health division, you will provide skilled nursing care to geriatric patients in the community. You will collaborate with a supportive team to implement care plans, assist in discharge planning, and ensure patient well-being.
Responsibilities
- Provide skilled nursing care and treatments to geriatric patients.
- Implement patient-centered care plans in alignment with available resources.
- Participate in developing patient discharge plans.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
