Job Description
We are looking for dedicated Licensed Practical Nurses (LPNs) to join our Home Health team at ChristianaCare. Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing patient discharge plans.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license, or compact state license.
