Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our dynamic team at ChristianaCare HomeHealth. Our organization provides exceptional support and resources for LPNs, including comprehensive OASIS-E training and a cutting-edge Electronic Health Record system. As part of our team, you will have the opportunity to work in geriatrics, home health, pediatrics, and wound care, delivering skilled nursing care and treatments to patients in the community.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients according to care plans.
- Participate in the development of patient-centered care plans and discharge planning.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.
