Job Description
ChristianaCare HomeHealth is looking for Licensed Practical Nurses (LPNs) to join the team in New Castle County, DE. The LPNs will provide skilled nursing care to geriatric patients in the community, implement patient care plans, and participate in discharge planning. Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.

$22.00 - $28.00 per hour
