Job Description

We are looking for a dedicated Registered Nurse (RN) Field Nurse to join our dynamic team at ChristianaCare HomeHealth. As a member of our team, you will provide skilled nursing care to geriatric patients in the community, ensuring goal-oriented and patient-centered care plans.

Responsibilities

• Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
• Develop and implement care plans that are goal-oriented and patient-centered.
• Work collaboratively with the team to develop discharge plans for patients.
• Be proficient in IV therapy or willing to train and perform IV functions.

Requirements

• BSN required; ADN candidates must obtain their BSN within three years of start date.
• Current DE RN license or compact state license.
• 2 years of HomeHealth RN experience within the last 12 months.
• Previous Home Healthcare and Med-Surgical experience preferred.
