Job Description
We are looking for a dedicated and experienced Registered Nurse (RN) Field Nurse to join our Home Care team. As an RN in HomeHealth, you will provide skilled nursing care to geriatric patients in the community, develop patient-centered care plans, and ensure the well-being of our patients.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Develop and implement care plans that are goal-oriented and patient-centered.
- Prepare patients for discharge and ensure a smooth transition.
Requirements
- BSN required; ADN candidates must obtain their BSN within three years of employment.
- Current DE RN license or compact state license.
- 2 years of recent HomeHealth RN experience.
- IV proficiency or willingness to learn.
