Responsibilities
- Conduct home visit physical therapy assessments and provide direct patient care.
- Documentation of patient progress using EMR systems.
- Collaborate with patients and families to develop personalized rehabilitation plans.
Requirements
- Graduated from an accredited Physical Therapy program.
- Valid Physical Therapy license in New York.
- Proficiency in EMR documentation.
- CPR Certification.
