Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Unencumbered Physical Therapy (PT) license for New York.
• Experience conducting home visit Physical Therapy assessments.
• Proficient in EMR documentation.
• CPR Certification.
