Responsibilities

• Conduct home visit assessments of referred patients and provide direct patient care.
• Assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
• Proficient in EMR documentation (Hello Note experience not required).
• Maintain a caring and attentive approach to patient interactions.

Requirements

• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• CPR Certification.
• Willingness to conduct in-person home visits in Brooklyn.
