• Conduct home visit assessments to evaluate medical conditions and functional abilities.

• Collaborate with patients, families, and healthcare professionals to ensure comprehensive care.

Requirements

• Graduate from an accredited Physical Therapy program.
• Hold an active Physical Therapy license for New York.
• Proficient in EMR documentation.
• Current CPR certification.
