Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Conduct home visit assessments, providing direct patient care and assessing medical conditions.
• Proficient in EMR documentation, with experience in Hello Note being a plus.
• Hold CPR Certification.
