Requirements

• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold an unencumbered PT license for New York.
• Conduct thorough home visit PT assessments and provide direct patient care.
• Proficient in EMR documentation.
• Hold CPR Certification.
