Responsibilities

• Conduct home visit assessments to evaluate patients' medical conditions and functional limitations.
• Provide direct patient care and develop rehabilitation plans.
• Document patient information using EMR systems.

Requirements

• Graduated from an accredited Physical Therapy program.
• Hold an unencumbered Physical Therapy license for New York.
• Proficiency in EMR documentation.
• CPR Certification.
