Licensed Practical Nurse MDS Coordinator

Job Description

Genesis Health is seeking a passionate and skilled Licensed Practical Nurse MDS Coordinator to join our committed healthcare team. As an LPN MDS Coordinator, you will play a pivotal role in the Resident Assessment Process by completing essential elements on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and proper nursing center reimbursement.

Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument (RAI) process in compliance with established guidelines for MDS completion, computer encoding, and electronic State transmission.
• Contribute to the development of personalized resident Care Plans, identifying resident needs and anticipated outcomes as mandated by Federal and State regulations.
• Provide training to new interdisciplinary team members on clinical content and electronic submission of relevant MDS components.

Qualifications

• Active LPN license in good standing.
• Prior experience in hospital, long-term care facility, or healthcare-related facility.
• Proficiency in general, rehabilitative, and restorative nursing practices, medical procedures, and knowledge of relevant laws, regulations, and guidelines.
• Computer skills including familiarity with Windows applications.
• Preferred experience with AccuMed software.
• Prior MDS Coordinator experience is a plus.
• Proficient in RAI/PPS processes.
• Current BLS/CPR certification (excluding ALFs/ILFs).
