Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

We are looking for a licensed practical nurse to join Genesis Health as a Minimal Data Set Coordinator. In this role, you will play a crucial part in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and nursing center reimbursement.

Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument (RAI) process following guidelines for completion and electronic transmission.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Train new team members on clinical content and electronic completion of MDS items.

Requirements

• Active LPN license.
• Previous experience in a healthcare facility.
• Proficiency in general, rehabilitative, and restorative nursing practices.
• Computer skills with familiarity in Windows applications.
• AccuMed experience is a plus.
• Prior MDS Coordinator experience preferred.
• Good knowledge of RAI/PPS processes.
• Current BLS/CPR certification.
