Job Description

Genesis Health is seeking a skilled Minimal Data Set Coordinator with an LPN background to join our team. As an integral part of our healthcare services, you will play a crucial role in the Resident Assessment Process by completing MDS and CAAs accurately to ensure optimal patient care and facility reimbursement.

Position Highlights

• Lead the interdisciplinary RAI process, ensuring compliance with completion guidelines, encoding, and electronic transmission of the MDS.
• Contribute to the development of individualized resident Care Plans, addressing resident needs and expected outcomes per regulations.
• Provide training to new team members on clinical content and electronic MDS completion.

Requirements

• Must hold an active LPN license.
• Prior experience in hospital or long-term care settings.
• Familiarity with nursing practices, medical procedures, and long-term care regulations.
• Proficiency in computer skills, including Windows applications.
• AccuMed experience preferred.
• Previous experience as an MDS Coordinator is a plus.
• Current BLS/CPR certification required.
